Family Health Assessment
A family health assessment is important in providing information related to a particular family, which may help a healthcare practitioner in making informed decisions. This may include the implementation of preventive strategies and health promotion interventions (Edelman, Mandle, & Kudzma, 2014). To assess a particular family, open-ended, family focused questions were developed which were based on different health patterns, which covered important healthcare aspects (Edelman, Mandle, & Kudzma, 2014). The family interviewed is the John’s family, which includes Mr. and Mrs. John who have been married for ten years and have three children. The father is forty-four years with the mother being forty years old while their three children’s ages are twenty, sixteen, and twelve. The father works as a driver at a local bus company while the mother is a homemaker with two children in school while the older one works as a teacher at a local public school.
Values/ Health perception
This involved an analysis of the different perspectives, which the family has on certain healthcare practices. The parents are obese which is attributed to the poor feeding habits in the family. This is a major concern to the parents whom their children largely depend on for guidance on healthy eating habits. The children are active in sporting activities, which has helped the kids to maintain a healthy weight. The mother suffers from diabetes as well as hypertension, which has led to some of the healthy interventions implemented by the mother to improve her health as well as the health of all members of her family. This has been achieved through the health education, which the mother receives from her physician. The family goes for regular checkups to determine their current health status with religion also playing an important role in healthcare.

Nutrition
The family has had poor feeding habits, which have led to the problems the parents are experiencing. Some of these eating habits include frequent eating without much consideration of the meals being taken some of which are unhealthy foods. The family shares the main meals together, which include breakfast, lunch, and supper. The family also enjoys a cup of tea every day after the supper meal before they go to sleep. The family believes that a proper nutrition plan involves eating regular meals instead of three large meals. Their meals largely constitute foods rich in proteins, which include sausages, meat, and beans. The parents also take beer and wine after the father comes from his job while the children take milk and juice when they come from school.

Sleep/Rest
Most of the family members receive 6 to 10 hours of sleep everyday with the exception of the father who comes late from his job and rises up early before everyone else to report to his job. The children go to bed early due to the following day where they are supposed to rise up early to attend school. However, the twenty year old is not subject to the house curfew set by the parents, which requires everyone to go to bed at 10:00 pm. This is not a problem to the children, as they do not have a problem sleeping early. This also helps the children in waking up early and refreshed to prepare for their day at school.
Elimination
The mother visits the restroom regularly, which is attributed to her diabetes problem. The mother has also constipation issues, which has led to her increasing her fiber intake in her foods. This problem has affected her since she was a little child which she later received recommendations to increase her fiber intake to help in addressing the problem. The father also regularly uses the rest room at night, which is attributed to the wine, and coffee, which he takes before going to sleep. The rest of the family members have no problem with their elimination process.

Activity/Exercise
Mr. John is always busy with his schedule at work, which leaves him little time to involve himself with exercise or sporting activities. During his free time, Mr. John likes to spend time in the house with his family and sometimes go fishing with his children. Mrs. John on the other hand has been recommended to exercise regularly due to her condition, which is aimed at managing her condition. This has enabled her to registering as a member at the local gym. The children are busy and active in school by engaging in sporting activities.

Cognitive
The family members are good both in remembering and grasping information, which is attributed to the children’s performance in school. The children do not need assistance with their homework since they are able to do it on their own. The family members are all able to hear, write, and read which makes the family satisfied with their cognitive process stating that they do not have a neurological or cognitive problem.
Sensory/ Perception
The family feel that their sensory perception is adequate and good. All the members’ senses are functioning effectively which include tasting, hearing, seeing, feeling, and smelling. However, Mrs. John’s sight has been affected by her diabetic condition, which has resulted in her using glasses to read. Regardless of this, she is able to perform all the other activities without them.
Self-Perception
The family reports having a high self-esteem besides their father who has been affected by his weight. Mr. John is obese which has lowered his self-image and self-esteem due to how the society perceives obese individuals, which is meant to imply that obese individuals love food so much and are unable to maintain their weight. Mrs. John is however very confident and loves herself so much which has been motivated after changing her diet and enrolling at the local gym. The children also love themselves very much and are confident which also encourages their parents.

Role Relationship
Mr. John is the sole provider of the family but also gets help from her first born who is employed as a teacher. Mrs. John is also involved in small businesses, which she uses the proceeds to help her husband in providing for the family. She also performs other important roles in the family, which include cooking and washing. Mr. John and his wife are responsible for making important decisions in the family with the wife being the disciplinarian in the family as she spends most of her time with the children while her husband is at work. The children help their mother in house chores when not in school.

Sexuality
John’s family believes that sexuality is a topic, which should be open and discussed as it has an impact on their health promotion strategies. This has helped the children in this family to have responsible sexual lives while in school and relating with other individuals of the opposite sex. Mr. John states that sex is very important between him and his wife as it helps in rekindling their relationship and increasing the bond they have as husband and wife. The couple is not afraid to display their affection in public or in front of their children and is confident that their children will be responsible in the future in terms of sexual responsibility.
Coping
The family deals with stress as a family, which has helped the family to overcome certain challenges, which have previously affected the family. One of the strategies involves having an open and effective communication, which is important during family meetings. This allows every member to participate in the discussions by providing his/her views and opinions related to the topic being addressed. The family is also religious and believes in involving a spiritual aspect in addressing some of the challenges faced by the family. This involves praying together during times of challenges, as they believe this is important in determining the outcome of the issue. The family also involves the church, family, and friends in times of challenges to provide support to them.
Wellness Diagnosis
From the information collected during the interview, one of the wellness diagnoses is preparedness for improved exercise patterns (Allen, Ryan, & Framer, 2013). The family is largely involved in healthy patterns, which have been achieved by their involvement in exercise. Majority of the family members are aware of the benefits of exercise on their health, which has led to the members involving themselves in exercise and sporting activities (Allen, Ryan, & Framer, 2013). The second wellness diagnosis is readiness for the family to access more information on health education due to their risk of developing lifestyle illness like their mother. The family conducts regular health checkups, which are meant to determine their current health status (Allen, Ryan, & Framer, 2013).
During these checkups, the family is able to receive health education, which is an indication that the family aims to live a healthy life. The parents have also played an important role in ensuring that their children are adequately equipped with information related to sexuality by being good examples to them (Allen, Ryan, & Framer, 2013). There are no major concerns in the family as the family has already showed their commitment in addressing any risky behaviors, which may have a negative effect in their health (Edelman, Mandle, & Kudzma, 2014).
